Functional rehabilitation system - Development and implementation of the comprehensive functional assessment system in rehabilitation with the use of foreign experiences and solutions

The overall perspective of the project ist the development and implementation of an assessment system for the Polish health care system. This assessment system is intended to improve the quality of medical rehabilitation in Poland by fostering the standardization of patients' functional assessment with the use of the International Classification of Functioning Disability and Health (ICF). The project will involve the formulation of the standards for functional assessment which will enable to describe the patients in a standardized and individualized way. For this ICF-based category profiles are formulated for the assessment in the area of inpatient and outpatient rehabilitation. These ICF-based category profiles are the base for a standardized procedure for support planning and intervention. Implementation will take account digital solutions for assessment by health care professionals (HCP) and self-assessment by patients. In this context, the method of ambulatory assessment will be tested in particular as possible solution for routine outcome monitoring in medical rehabilitation. Different patient groups (e.g. individuals with musculoskeletal, cardiopulmonary and neurological health conditions) and different types of rehabilitation services (e.g. inpatient and outpatient rehabilitation, acute and early postacute rehabilitation) are considered.
